504/IEP Accommodations Form

As a result of our experience with the school systems and
the feedback you have given us over the past year, we have
developed the following 504/IEP Accommodations Form. It is
recommended that you take this form to your 504/IEP meeting
and use it to develop practical interventions for your child.
We suggest that you have the school make a copy of the
completed form for their records.
Unfortunately, at this time, we are unable to provide this form on our
web site. If you would like a copy of this form, please email Alan at
